Information:

• Cost: $200 all inclusive

• For UGBC students Deposit is $50.00. It is required to hold your reserved spot (unless you make other arrangements with Josh).  Checks should be written to Union Grove Baptist Church Youth. The registration form, medical form, and deposit are due by March 28^th, 2021.
• We will be doing fundraisers to cover the rest of the part of camp. Each participant will need to be at the fundraisers to have the money cover their cost. If they do not show up, the fundraisers will not go towards their account.
• We will be leaving after the Sunday Morning Worship on July 18^th, and return in the evening on Saturday, July 24^th. Your contribution will cover the cost of food, activities, zip lining, Ark Encounter, travel, lodging, and all other expenses.

• By signing this form, I am consenting to follow the direction of accompanying UGBC adult leadership, and I consent to allow the accompanying adult leadership to undertake any disciplinary measures that may be necessary as well as address medical issues that may arise. If a student is not 18 years old, then a parent or guardian needs to sign the form too.
